Once you've scheduled an appointment with us, you'll receive an email to sign up for Prelude Connect, our patient portal. Please fill out all the forms in the portal ahead of your visit, including the medical release form. Having access to your medical records will give us the insight we need to make your first visit as successful as possible.
Please bring the following to your first appointment:
During your visit, you and your doctor will review your medical history, discuss your family-building goals, and talk through treatment options. You are free to ask questions and share any information you think is important for us to know.
Then, your provider will determine the best treatment plan and order any necessary diagnostic testing for you and your partner (if applicable). A member of our care team will reach out regarding next steps, typically via the Prelude Connect portal.
Our team will work with you determine scheduling and outline what you can expect at each step of your treatment plan. We want you to feel comfortable and cared for during your time with us, so please reach out with questions or concerns at any time.
